Homocystinuria (HCU) is an inherited disorder of the metabolism of the amino acid methionine due to a deficiency of cystathionine beta synthase or methionine synthase. It is an inherited autosomal recessive trait, which means a child needs to inherit a copy of the defective gene from both parents to be affected. === Lawsuits === In April 2012, Johnson & Johnson (J&J) and its subsidiary Janssen Pharmaceuticals Inc. were fined $1.2 billion for downplaying multiple risks associated with risperidone. The verdict was later reversed by the Arkansas state supreme court. In August 2012, J&J agreed to pay $181 million to 36 US states to settle claims that it had promoted risperidone and paliperidone for off-label uses including for dementia, anger management, and anxiety. In November 2013, J&J was fined $2.2 billion for illegally marketing risperidone for use in people with dementia and paying kickbacks to prescribing physicians and nursing home pharmacies. In 2015, Steven Brill wrote an investigative journalism piece about J&J in The Huffington Post focused on J&J's marketing of risperidone. J&J has faced numerous civil lawsuits on behalf of children who were prescribed risperidone who grew breasts (a condition called gynecomastia); as of July 2016 there were about 1,500 cases in Pennsylvania state court in Philadelphia, and there had been a February 2015 verdict against J&J with $2.5 million awarded to a man from Alabama, a $1.75 million verdict against J&J that November, and in 2016 a $70 million verdict against J&J. === Challenges === Despite its promise, relatively little silk has reached the clinic, and the reasons are specific. Foremost among them is variability. Silk is a farmed natural product, so its composition shifts with the silkworm strain, its diet and its rearing conditions, and this batch-to-batch variation is difficult to reconcile with the strict specifications required of a medical device. Processing introduces further variation: the duration of exposure to the dissolving salt and the amount of heat applied both affect the molecular weight of the regenerated protein, and with it the strength and degradation of the final product. Sterilisation presents a further difficulty, since standard autoclaving alters the structure of the protein, and a gentler method must be identified and validated for each format. Controlling the degradation rate precisely, scaling laboratory processes to industrial volumes, and meeting the regulatory requirements for an implanted device are each demanding in their own right. These engineering and manufacturing obstacles, rather than any deficiency in the biological performance of silk, are the main reason that most of it remains in the laboratory.

According to the European Monitoring Centre for Drugs and Drug Addiction (EMCDDA) it has been detected in 25 European countries with the majority of drug seizures in Poland and the largest quantities in the Netherlands.
